The Festering Chasm: Head west from the teleporter. Keep running until you reach a cave with a Cracked Stone Grate inside. This is also reachable from the Fungi of Si'ned VII level, but you might not notice it the first time around. Once you do, follow the rock wall to the right. Interacting with this teleporter teleports you to a normally unreachable island off in the distance, the Putrid Anomaly. You don't even need to get every upgrade, although having more mobility will certainly make life easier. Just scan it later. While you'll see plenty of these in The Verdant Wilds of Zyl, I recommend another area that isn't filled with tall grass in order to scan these guys.
